Okay, brltty is doing everything right, but, when all is said and done, can't 
succeed. The final error is this one:

   cannot create device: /mnt/lib/brltty/rw/vcsa: Operation not permitted

It seems, therefore, that the kernel on that rescue CD doesn't support the 
mknod function so there isn't much brltty can do.
