You may try to edit the preference file i.e. where BRLTTY saves your menu selections.  Its default location is /var/lib/brltty/brltty.prefs.

In particular, you might want the following options:

say-line-mode immediate
autospeak yes
autospeak-selected-line yes
autospeak-selected-character yes
autospeak-inserted-characters yes
autospeak-deleted-characters yes
autospeak-replaced-characters yes
autospeak-completed-words yes

I don't know how robust BRLTTY is with hand editing this file though.
