After struggeling a bit, I got the structural navigation to work within navigation mode. Although spaceLeft+2578 gives a sound from the Actilino device, the keystroke is passed along to BRLTTY, and structural navigation is selected. Beeing in the navigation mode, I can use e.g. h to go the next heading.
