Does it crash on wakeup, on USB disconnect, or on USB reconnect?

Is this on Windows or on Linux?

If it's on Windows, are you able to verify if it also happens on Linux?

If it's on Windows, are you using libusb or libusb-1.0, and which Windows 
version are you using?
