It has been discussed at length on the eyes-free list. I think it can be found in the 5.0.4 release notes, however it was also communicated by V ictor Tsaran of Google in that same list. I don't know how an app like shouter retrieves the required information. I think the basic idea behind screenreaders like TalkB ack is that it only provides information that can be obtained through standard interfaces and not to go hunting for that info e.g. By processing the entire screen.
Just guessing.
