So I guess it's a bug that's never been fixed (or maybe even known about).

Remember that it also happens for me (so on x86_64) right after a reboot. I've
been trying to think of what might be different between starting brltty as part
of a system boot versus starting it interactively. So far, I have no ideas.

espeak_Synth() to speak, and espeak_Cancel() to mute.
