Do most keys work as they should?

Are you referring to keys not working correctly when using brltty directly or 
when nvda is in control?

If it's when nvda is in control, are you able to test brltty directly in a 
text window to see if the problem is there too?

Are you also seeing the COM3 access problem?

If yes, what I don't understand is how that problem can exist concurrently with 
apparently being able to use the device at least somewhat.

Sure. The more detail, the better.

Can you also use Command Learn Mode to check out what the problematic keys 
seem to be mapped to?
