Are you asking about what brltty does, or are you writing your own software and 
wanting to get some advice on what you should have it do?

Brltty does one of two things. It has a preference setting for "Expand Current 
Word". if set to Yes, the word where the cursor is is shown in uncontracted 
six-dot braille. If set to No, the cursor can only go onto the first cell of a 
contraction. So, in your "according" example, pressing the cursor routing key 
above the "c" will route the cursor to the "a".
