[BRLTTY] patch for open as a macro

As it turns out, I haven't really been happy with the names open() and close()
anyway but there was never a good enough reason to make it worth renaming them. 
Unless anyone objects, I think I'll use this opportunity to rename them to
construct() and destruct().
