I see the following in the log file you attached:

>brltty.exe: checking braille device: serial:COM3

>brltty.exe: CreateFile error 5: Access is denied.
>brltty.exe: cannot open serial device: COM3
>brltty.exe: braille driver initialization failed: bm -> serial:COM3
>brltty.exe: braille driver not found

I'm not a Windows user so I've no idea wqhy COM3 would be inaccessible when 
using brltty-4.2 but accessible when using brltty-4.1, but it looks to me like 
that'd be the problem.
